Care Planning

The main screens for Care Planning are accessed by opening a Person record and selecting the Care Plans tab.

Under this selection a number of tabs are available, including:

Plan Summary

An overview of each Care Plan, displaying the current situation, expected outcomes, and actions

Personalised Care and Support Plans

All Care Plans for the selected Person

Assessments & Plans

​Incomplete and completed Care Plan Forms

Care Reviews

All Care Reviews for the selected Person

eMAR

Electronic Medicines and Administration

Risk Plans

All Risk Plans for the selected Person

Diary & Preferences

All Preferences that have been recorded for the selected Person for different types of care. Displays the next 7 days of scheduled care actions for the selected Person, based on the recurrence set in Regular Care

Regular Care

Regular care delivery items for the selected Person, and the required recurrence of that care

Bookings

The Bookings tab is available where 'Mode of Care Delivery' is set to Domiciliary and shows any bookings allocated to the Person selected. It also displays a list of the Regular Care for that Person, indicating which of those have been marked as completed by the attending staff member(s).

Handovers

Daily Care records which have been flagged as relevant to the Handover

The 'Mode of Care Delivery' chosen in Care Provider Setup will determine which features are visible under a Person's Care Plan (see Configuring the Care Cloud Mobile App).
