Charting Per Care Plan ADL

This can be used to make Users aware that Charting is in place in the Care Plan and for reporting in Advanced Search:

For Care Plans which have an ADL in the reference data with charting enabled , that Care Plan will then have the corresponding charting options available:

  • Monitoring/Charting Required For Foods
  • Monitoring/Charting Required For Repositioning
  • Monitoring/Charting Required For Post Falls
  • Monitoring/Charting Required For Fluids
  • Monitoring/Charting Required For Welfare
  • Monitoring/Charting Required For Distressed Behaviours

For example to set up a Care Plan for Fluid Charting:

Update an existing or create a new ADL for Fluid (name as you like), and choose "Monitoring/Charting Required For Fluids":

This will the give the option per Person (within Care Plans linked to this ADL) to indicate that they currently require Charting for Fluids:

Care Plans linked in this way, that are Authorised, then show icons in the Person Banner to show the charting which is active. This is also shown in the Mobile App.

Care Monitoring Charts

This can be used to see a visual of the Care Recorded today, to review this and to save that information. This is currently available for Repositioning Care only.

Previously created Chart information can be viewed here:

The Chart information can be created by creating a new record for the date of the users' choice:

This will produce a PDF which is saved within the system, which can be marked as Reviewed:

Page 1 has Planned Care (record is planned below but not recorded as done)

Page 2 has Ad-Hoc Care recorded:

The reviewer can then 'Mark as Reviewed' via the 'More Actions' feature:
