Question Catalogue

​The Question Catalogue is where your organisation can create, modify, and store Questions that can be added to Documents which will be used to create Forms, and promotes consistency of wording across multiple Forms. Care Cloud comes with some questions included in the catalogue, but you may wish to add additional ones relevant to your organisation. Also updates are only needed in one place and all instances of the question will benefit. Adding and changing questions in the Question Catalogue ensures consistency across all the forms and if a change is needed it will be updated throughout the system.

Caution, if you change a Question in the Question Catalogue, you need to be mindful that it won't just affect the Form you're working on, but all other Forms using the same question.

To access the Question Catalogue:

  1. Navigate to Settings > Forms Management > Question Catalogue.

The 'Question Catalogue' page is displayed:

This displays all of the available questions in the grid.

  1. Use the Drop-Down List to filter specific question types.

Creating a Question

To create a new question within the Question Catalogue:

  1. Select the Create new record button on the toolbar.

The 'Question: New' page is displayed:

  1. Complete the fields on the page. Mandatory fields are marked with a red asterisk.
  • Question – Text of the question as it will appear on the Form.
  • Sub Heading – Text of a sub heading for the question if required.
  • Question Type – Define the type of question, e.g: Date, Multiple Choice, Numeric, Text. There are multiple fixed question types in Care Cloud that will display as a drop down list. This is the most crucial step.
  • Identifier – Unique number for the Question that is automatically generated by Care Cloud when saved.
  • Local Identifier – An optional local identifier that can help identify questions associated to a particular form or process, e.g. Risk 1 for all questions used on the Risk 1 form.
  • Read Only – Indicates if the Question is read-only when viewed in a Form.
  • Locked – When the Question is locked (Yes) you cannot edit certain details or delete the Question because it is in use on a published form.
  • Reporting Identifier – Text that can be used to identify a Question for reporting purposes
  1. Select the Save button on the toolbar.

Question Views

Question Views allow you to capture more than one answer for the same question, e.g. View of the Carer, View of the Client, however your Question must meet the following criteria:

  • The Question Type is one of the following:
    • Date
    • Decimal
    • Numeric
    • Short Answer
    • Yes/No.
  • Capture Multiple Views in the Question Details field is set to Yes.

To create a new Question View:

  1. Navigate to the Question within the Question Catalogue.
  2. Select Menu > Related Items Question Views.
  1. Select the Create New Record button on the toolbar.

The 'Question View: New' page is displayed:

  1. Complete the fields on the page. Mandatory fields are marked with a red asterisk.
  • Question Catalogue – Question for which the Question View is created.
  • Name – Label for the Question View as it is displayed in the Form.
  • Display Position – Order in which the current Question View is displayed.

  1. Select the Save and Close button on the toolbar.
  2. Repeat as desired for as many Question Views as needed for the Question.

Question Mapping

Question mapping is a feature in Care Cloud that automatically fills Question fields where the information has been previously entered on the system. This prevents the need to manually enter duplicate answers and maintain consistency across Forms created for the same Person Record. Automatic and manual mapping can be applied in Care Cloud.

Automatic Mapping

Automatic mapping can be applied when a Question reoccurs, e.g. the most recent answer to the same question is pulled from any existing Form that was created for the same Person Record.

Care Cloud will run automatic Question Mapping when the relevant section of the Form is opened for the first time.

To enable automatic mapping of a Question:

  1. Select Settings > Forms Management > Documents.
  2. Select the relevant Document.
  3. Select the Sections tab.
  4. Select the Questions tab.
  5. Select the relevant Question.

You will be presented with the following page:

  1. Set the Default SDE Enabled field to Yes.
  2. Select the Save and Close button.

Now when a User creates a Form, Care Cloud will check existing Forms for the same Person Record. If it finds the same Question on other Forms, it will automatically populate the field with the most recently modified answer.

Automatic mapping will be disabled if Mapping Mode is set to Do not copy answers when new assessment is created on the Document Record.

Manual Mapping

Manual mapping allows you to map a Question with a Question added to a Document of the same Category.

Questions can be mapped manually if the following criteria are met:

  • Target Question is in a Document of the same Category.
  • Target Question is of the same Question Type.
The following Question Types cannot be mapped: CMS, Flag, and Hyperlink.

To manually map a Question:

  1. Open the relevant Document record.
  2. Select the Preview Document button.
  1. Select the Manage SDE Maps button beside the Question that you want to map.

The SDE Maps for Question screen is displayed:

Select one of the two buttons depending on your aim of the mapping:

  • Create Map From this Question – Care Cloud retrieves the answer for this Question and saves it to another specified Question
  • Create Map to this Question – Care Cloud retrieves the answer for another specified Question and save it to this Question

Follow the example below to create a map to this Question.

  • When do you want to run this mapping: Execute this mapping when new assessment is created (mapping is only applied when the relevant section of the Form is opened for the first time).
  • Two way Map: No (select Yes if you want the mapping to work in both directions between Questions).
  • Is Conditional: No (select Yes if you want the mapping to be executed if a plugin rule is met, this will display the Rule field).
  • Document Category: specify where the document to map to is stored, in this example the target Document is in Person Form.
  • Document: Specify the target Document that contains the relevant Question.
  • Section: Specify the Section within the document and click the relevant Questions to map to.

Once you have completed all the fields, select Save and your new SDE map will appear on the Document Question Mapping.

Any mappings in place for a Question that is answered by another mapping are automatically triggered.
