Creating a Person Discharge

In the context of a care home, "discharge" refers to the process of a resident leaving the care home. This can occur for various reasons, and it involves careful planning and coordination. Here's a breakdown of the out-of-the-box discharge reasons:

Change of Funding

Sometimes, financial situations change and a resident can no longer afford the care.

Death

Unfortunately, in some cases, discharge might involve the resident passing away.

Gone to Another Home

The resident or their family may choose to relocate to a different care home or area.

Gone to Hospital

A resident's needs might change, requiring a different type of care than the current care home provides. For example, they might need more specialised medical care or end-of-life care.

Returned Home

A resident's health may improve to the point where they no longer require the level of care provided in the home. They might return to their own home or a less intensive care setting.

To create a Person Discharge record in the system:

  1. Navigate to People > People > People We Support.
  2. Open the Person record in the usual way.
  3. Select Menu > Related Items > Discharge. The 'Person Discharges' sub-page is displayed.
  4. Select the Create new record button on the toolbar to create a Discharge. The 'Person Discharge: New' page is displayed.
  5. Complete the fields on the page, mandatory fields are marked with a red asterisk *:

Field

Description

Leave Date and Time*

Select the date and time the Person is to be discharged.

Discharge Reason*

The reason why the Person has been discharged.

Funding Authority Notified

If a Funder is used, have they been told.

Responsible Team*

The team responsible for managing the record.

Discharged To

If they are being discharged to another establishment, family or hospital.

Date Of Death

If the resident passed away, enter the deceased date.

  1. Select the Save button on the toolbar.

The Discharge Process

The discharge process may involve several steps. Some of the things to consider:

  • Assessment: A thorough assessment of the resident's current needs is conducted.
  • Planning: A discharge plan is created, outlining the necessary steps for a smooth transition. This may involve coordinating with other healthcare professionals, family members, and social services.
  • Coordination: Arrangements are made for transportation, any necessary equipment, and ongoing care.
  • Documentation: Detailed records of the resident's care and discharge are maintained.

Essentially, a person's discharge from a care home is a process that ensures the individual is moved to a setting that best suits their current needs, with proper planning and coordination.
