Recording Assessments

Once a Care Plan has been created, assessments can be recorded for the Person in the Assessments & Plans tab.

This screen will show any Assessment Forms that exists for the Person in the appropriate section as follows:

To create a new Assessment:

  1. Select the Create new record (+) button within any of the sections:
  1. Complete the General section about the Assessment.
    1. The Form Type look up allows selection of any of the available Assessment Types which can be applied. A large number of these come as standard with the product, but more can be added by a System Admin (see the Care Planning and Recording article for more information).
  2. Once the General section is complete, select the Save button on the toolbar.
  1. Additional options become available on the toolbar. Select the Edit Form (Pencil icon) button to open the Assessment:
  1. Complete the Form and select the Save or Save & Close button on the toolbar.

Form Authorisations

To close or sign off an Assessment, the System User is required to have the Can Authorise Form setting in their record, set to Yes.

This can be found by opened the record via Settings > Security > System Users, then opening and navigating to the Settings section of the System User record.

If an Assessment has previously been completed for the Person, answers can be copied into a new Assessment of the same type. An Assessment may copy all previous answers, none, or only those selected. This can be configured on the Form itself (see the article Care Planning and Recording for information).
